Your first visit will consist of a review any information provided by your general dentist, your medical history and medications, a clinical examination and required x-rays and periodontal charting. Time will be available for your questions.

All of this helps us personalize a treatment plan for you and communicate effectively with your general dentist, other specialists and insurance companies on your behalf.

Please bring a complete list of your current medications and arrive at the office about 15 minutes early so that we can collect and review your information. For your convenience we have included the documents we will ask you to fill out upon arrival:

Patients are responsible for all fees associated with their consultations and care.

CONSULTATION FEES:
New patients to the practice must come prepared to pay a consultation fee, along with any radiographs required during the appointment in full, that day. We accept payments by cash, cheque, VISA, Mastercard and Debit.

TREATMENT FEES:
Insurance plans are agreements between patients and their insurers and do not affect the responsibility of the patient for all fees incurred.

However, after patients have had their diagnostic work completed with us, we work diligently to assist them in determining what their insurance company will pay towards recommended treatment (pre-determinations). If pre-determinations are in place prior to treatment appointments, patients may opt to assign their benefits to their office account and pay the balance owing on the day of treatment. Without pre-determinations, patients pay in full for treatment and insurance claims will be submitted by us made payable to the patient.

In certain circumstances, we can offer plans to patients willing to arrange pre-authorized, dated payments on their accounts preferably through credit card.
